The prison is run by HMPS as part of the North West prison region. It has a capacity of 847 and the #1 governor is Andy Lund, who has been in charge since August 2022.

Garth, near Leyland in Lancashire, is a Category B training prison for adult men serving long sentences, with capacity for around 850.

A full inspection in 2024 found poor safety, ready availability of drugs, staffing and retention problems, and uncomfortably hot, poorly ventilated cells. A follow-up review in June 2025 found only minimal improvement, with roughly a third of random drug tests still coming back positive. Garth has also been badly affected by drone deliveries of contraband, with inspectors describing prisoners burning holes in cell windows to bring packages inside; reinforced window grilles are being fitted as part of a national £35 million security programme in response.
